6. Community First (managed care in Bexar county): To enroll as a contract provider, write a letter of intent and fax it to Provider Relations. The letter should state your intention to be a contract provider for:
*Commercial HMO,
* Medicaid STAR,
* Childern's Health Insurance Plan (C.H.I.P).
A copy of your most recent W-9 tax form should be faxed along with your letter.
